The ABSORB III Imaging substudy biomechanical analysis revealed that patients randomized to Bioresorbable vascular scaffolds (BVS) compared to everolimus-eluting stents (XV) had no significant differences in post-stent intravascular ultrasound characteristics or underexpansion. However, BVS was associated with a significantly greater proportion of stents with low mean wall shear stress (WSS) and lower WSS values across the stent compared to XV. Low stent WSS was linked to lower contrast velocity, less residual plaque, less stent underexpansion, higher eccentricity index, and BVS placement. Independent predictors of greater length of low WSS were hypertension, BVS placement, lower blood velocity, less residual plaque, and less stent underexpansion. The study suggests that the design of BVS, particularly the thicker strut size, may predispose post-stented vessels to low WSS, potentially mediating adverse healing conditions and increased risk of scaffold thrombosis.
